You have cracked JEE and secured your rank. Take a breath,you actually did it. But before you completely zone out, there’s a harsh reality check waiting on the JoSAA portal: the paperwork.
I have seen students who solved the hardest physics problems freeze up because their admission got stalled over one stupid document. The main culprit? The JoSAA Medical Certificate.
Everyone leaves it till the last minute. The panic is real when a verifying officer suddenly rejects the format. I’m not letting a missing signature or a wrong stamp mess up your hard-earned IIT or NIT seat.

Why Does JoSAA Need a Medical Certificate?

They also need to check for specific conditions, like severe colour blindness. Some branches, like Mining Engineering, Petroleum Engineering, or Geology, have strict physical requirements. That is the only reason this document exists. It is not an interrogation; it is just a standard health check.
Step 1: Downloading the Correct Format
You must strictly use the official medical certificate format for JoSAA counselling provided by the authorities.
- Go to the official JoSAA 2026 website.
- Look for the “Business Rules” or “Certificate Formats” section in the downloads area.
- Scroll down to the annexures. You will usually find the Medical Certificate format near Annexure 7.
- The exact annexure number may change this year.
- So, check the index.
- Print it out on a clean, white, A4-sized paper. Do not edit it digitally. You need a hard copy to take to the doctor.
Step 2: Who Can Actually Sign This Certificate?
The certificate clearly states it must be signed by a Registered Medical Practitioner (RMP).
What does that mean for you? It means you do not need to stand in a massive queue at a crowded government civil hospital if you don’t want to. You may visit your family doctor, a local doctor, or a private clinic.
Make sure the doctor has an MBBS degree or higher.

If the doctor practices Ayurveda (BAMS) or Homeopathy (BHMS), verifying officers often reject the certificate. Stick to an allopathic MBBS doctor to be 100% safe.
Step 3: How to Fill the JoSAA Medical Certificate
When you walk into the clinic, here is exactly what needs to be filled out. Do not leave any blank spaces!
- Personal Details: Write your Name, JEE Main/Advanced Application Number, Age, and Gender in your own handwriting. You may also ask the doctor to fill it. Keep the handwriting neat and consistent.
- Physical Parameters: The doctor will measure your height, weight, and take your blood pressure.
- Vision Test (The Biggest Source of Panic): The doctor will check your vision. If you wear spectacles, do not panic! Wearing glasses is completely fine. The doctor will simply note down your vision “with glasses.” Crucial Note on Color Blindness: The doctor will test you for color blindness. If you are color blind, the doctor MUST mention it. While colour blindness will not affect admission to Computer Science or Electrical Engineering. Some older IITs have strict rules for Mining and Metallurgy. Honesty here is critical so you don’t face issues during physical reporting.
- Blood Group: If you don’t know your blood group, you will need to get a quick 10-minute prick test done at a pathology lab. The doctor needs to write this down accurately.
- Past Medical History: Unless you have a severe chronic illness, major recent surgery, or a highly restrictive medical condition, this section is usually filled with a simple “NIL” or “None” by the physician.
Step 4: The Most Important Part – The Doctor’s Seal
The verifying officers sitting behind the screens at JoSAA look for three specific things at the bottom of the page:
- The Doctor’s Signature.
- The Date.
- The Official Seal/Stamp. The stamp is the ultimate proof of authenticity. This rubber stamp must contain the Doctor’s Name and, most importantly, their Registration Number. Without the registration number, the certificate is considered just a random piece of paper. Before you leave the clinic, double-check that the ink is visible and the number can be read clearly.
The Final Upload Checklist
Once you come back home with the signed document, your job isn’t quite finished. JoSAA has strict online upload guidelines.
- Scan it properly: Do not just take a shaky photo with your phone while holding it in your lap. Put the paper flat on a table in good lighting. Use a proper scanning app (like Adobe Scan or Microsoft Lens) to create a flat, high-contrast digital copy.
- Format and Size: Save the scan as a PDF file. JoSAA usually restricts file sizes (often between 50 KB to 500 KB). If your PDF is too large, use a free online PDF compressor. If it’s too blurry after compressing, rescan it. The text and the doctor’s stamp must be highly legible.
- Keep the Original Safe: You are uploading the scanned copy now, but when you finally travel to your allotted IIT or NIT campus in August for physical reporting, you MUST carry the original physical copy with you. Put it in a clear plastic folder right now so you don’t lose it or spill coffee on it.
